•Add a Layer Mask with Hide All Attribute
             Layer&gt;Layer Mask&gt;Hide All
•A Black thumbnail will appear on the target layer in the Layers Palette
•Select a Soft Brush
•Set the foreground color to white
•Make sure the Layer Mask is selected by clicking on it in the Layers Palette
•Start brushing where you want to hide wrinkles or smooth the skin
•You can vary the opacity of the brush to get the exact effect you need.  
•You can also play with the opacity of the layer

The trick here is that you have to add noise back in where you blur the skin.  Otherwise it just looks fake.

The values of the filter parameters will vary according to the image you’re working with.  These values are starting points for a snapshot sized portrait.  They look radical when you’re doing them but remember, most of this effect will be covered when you apply the layer mask.  You will be uncovering the effect when you paint the Layer Mask with white.  

In my experience, it’s easy to overdo this effect.  You get used to looking at the effect and you can’t tell when you’ve gone too far.  Make your changes and then walk away from it for a while.   When you come back in an hour or so, you’ll be better able to judge how it looks.

Also, when working on wrinkles and dark circles, I don’t take them all the way out, I just reduce them to a degree.  I remove temporary blemishes and blotchiness completely.  But permanent marks or moles I only reduce.  

I use this effect so often that I’ve taken this procedure and made it into my own action.  That way when I need it, I simply press a function key.  It’s easy to create an Action, you just make a recording of what you’re doing.  There are lots of web sites that will show you how.
